This Event Participation Waiver (“Waiver”) is made effective upon signing, by and between the attendee, herein referred to as “Participant,” and Haute Healing Oasis, the Center for Consciousness located in Stamford, Connecticut, and the hosting practitioner (“Practitioner”).
The Participant acknowledges that participation in the event involves inherent risks, including but not limited to the risk of personal injury, psychological stress, and damage to personal property. The Participant voluntarily agrees to participate in the event, fully aware of the risks involved, and assumes all risks of injury, illness, damage, or loss that might result.
The Participant, for themselves and their heirs, executors, administrators, and assigns, hereby releases, waives, discharges, and covenants not to sue Haute Healing Oasis, the Center for Consciousness, the Practitioner, their officers, directors, employees, volunteers, agents, and representatives from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, injury, or expense, including death, that may be sustained by the Participant, or to any property belonging to them, as a result of participation in the event, whether caused by the negligence of the releasees or otherwise, to the fullest extent permitted by law.
The Participant agrees to comply with all applicable health and safety guidelines and instructions provided by Haute Healing Oasis, the Center for Consciousness, and the Practitioner during the event. This includes adherence to local health regulations and guidelines set forth by Stamford, Connecticut.
The Participant acknowledges that they have carefully read this Waiver, understand its contents, and are signing it of their own free will. The Participant recognizes that this Waiver includes a release of liability and a waiver of the right to sue Haute Healing Oasis, the Center for Consciousness, the Practitioner, and other parties involved in the event. The Participant understands that this Waiver is a condition of participation in the event.
Should any portion of this Waiver be held invalid or unenforceable by a court of competent jurisdiction, such invalidity or unenforceability shall not affect the remaining portions, which shall continue in full legal force and effect.
By signing below, the Participant agrees to the terms outlined in this Waiver, photo release to take pictures during the event, and confirms their understanding and acceptance of the risks and conditions associated with participating in the event.
